Relapse Records on yhdysvaltalainen metallimusiikkia julkaiseva levy-yhtiö, joka perustettiin vuonna 1990 Philadelphiassa. Levy-yhtiön alkuaikoina sen yhtyeet olivat pääosin erilaisia extreme metal -yhtyeitä, mutta myöhemmin se rupesi julkaisemaan myös kokeellisempien yhtyeiden musiikkia. Suomalainen Amorphis kuului pitkään Relapse Recordsin yhtyeisiin.
